Register on Logbox as a patient if you would like to share your
medical aid and contact information with us electronically.

We look forward to meeting you. Please make yourself familiar with our Patient Agreement Policy and Billing Policy before making an appointment.

You are welcome to make an appointment by making use of one of the options below:

  1. Call the practice on 044-001 1133 to request an appointment
  2. Email us at admin@pletturology.co.za to request an appointment
  3. WhatsApp a request to 079 769 3182.
    (Please note this number is for appointments and admin queries only. DO NOT send any medical  urgent requests to this WhatsApp number)
  4. Complete the form below and we will get back to you as soon as possible:

Once you have booked an appointment we will send you a link to Logbox, where you can upload all your contact and Medical Aid details so you will not have to fill in more forms on arrival. You can also go direct to Logbox from this link.  

Many medical practices across the country use Logbox, and once you have registered on the site, whenever you go to a new health care provider they will be able to upload your contact information automatically just using your name or ID number
